Requesting Transcripts
- Complete the Transcripts Request Form
- Complete the Transcript Request form and pay the applicable fees at the Business Office in Building C Room 150,
Monday through Friday from 8:30 a.m. to 4:30 p.m., except for official holidays
- Submit the validated Transcript Request form along with your cash receipt to the Registrar’s Office
- Requests are processed within two working days of receipt
Requesting Transcripts by Mail or Fax
- Download and complete the Transcript Request Form
- To pay by credit card, download and complete the Credit Card Authorization Form.
- Fax completed forms to (562) 902-3306
- Or send in a check, money order or cashier’s check payable to SCUHS along with the completed Transcript Request Form to:

Once payment has been approved by the Business Office, transcripts requested by mail or fax will be processed within two working days of receipt.
Transcript fees
- $15 per transcript
- $10 per transcript if you are currently a member of the Alumni Association in good standing
- $5 per transcript for current students
